The center provides Uber/Lyft to those who don’t drive. Also, they offer exercise, art, and other activities for their seniors to promote better health at no cost to the patient.

Role:

• Compensation: Base $270,000 (negotiable) + up to 20% of base salary in bonus’
• Primary Care – Outpatient
• Value-Based Care Model
• Average patients per day: 10-12
• Panel size: Max of 650
• Shifts: Monday-Friday 8a-5p

• Physicians have their own care team with MA/scheduler/ referral specialist and full support from leadership
