• Active DEA with Tennessee address
• DEA Schedules II, IIN, III, IIIN, IV & V required
• Active NCCPA certification
• ACLS and PALS required
• Mentorship/proctorship may be required for new graduates

CREDENTIALING REQUIREMENTS

• MMR and Varicella documentation or positive immunity titers
• Two-step TB test within the past 12 months
• QuantiFERON Gold accepted per facility requirements
• If TB positive: chest X-ray within the past 3 months
• Case logs from the previous 2 years
• Preeclampsia-Eclampsia CME required upon initial appointment and every 2 years for reappointment
• Must meet all facility credentialing and privileging requirements
