About the Role

Join a growing NP-led healthcare organization dedicated to transforming primary care for older adults in underserved communities. You’ll provide full-scope, patient-centered care with the time and resources to focus on prevention, chronic condition management, and meaningful relationships.

Responsibilities

• • Provide comprehensive primary care and chronic disease management for adult and senior patients

  • Coordinate care and support for highly complex dialysis patients (non-nephrology role)

  • tConduct and document Comprehensive Health Assessments and Health Risk Assessments with a focus on accuracy, holistic care, and specific clinical coding

  • Administer diagnostic tests and preventive screenings

  • Develop personalized care plans and educate patients on disease management and wellness

Collaborate with an integrated care team to ensure continuity, quality, and value-based outcomes
