Nurse Practitioner – Community-Based Care

Location: Arizona (Commutable to Maricopa County)

We are seeking a Nurse Practitioner to provide comprehensive primary and acute care to patients in Assisted Living, Memory Care, Hospice, and Group Home settings throughout Maricopa County. This is a weekday role with no nights, weekends, or call. Daily patient volume varies based on acuity and visit type (intake vs. follow-up).

Responsibilities

• Provide primary and acute care to patients in community-based settings

• Order, interpret, and evaluate diagnostic tests

• Deliver patient education, counseling, and disease prevention guidance

• Collaborate with an interdisciplinary care team

• Develop, document, and update individualized care plans

• Prescribe medications and treatments within scope of practice

Requirements

• Master of Science in Nursing (MSN)

• Minimum of one year of Nurse Practitioner experience, or equivalent clinical background

• Current, unrestricted Arizona RN and NP licenses

Compensation & Benefits

• Salary up to $145,000 annually

This position requires local travel within Maricopa County to Assisted Living, Memory Care, Hospice, and Group Home settings.
