This value-based model emphasizes continuity, accessibility, and outcomes–delivered in welcoming neighborhood clinics designed to feel more like a community than a traditional medical office.

Patients benefit from transportation assistance (Uber/Lyft for those who don’t drive) and no-cost wellness offerings such as fitness, art, and social activities–removing barriers to care and improving overall health.

• 12-14 patients per day on average
• Capped panels (~600)
• Monday-Friday, 8:00 AM-5:00 PM
• Extremely light, shared call rotation
• Your own dedicated care team (MA, scheduler, referral support)

• Patient Population: Seniors with multiple chronic conditions

Compensation & Benefits

• Base Salary: $130,000+
• Bonus: Up to 10% of base (value-based incentives)
• Total Potential Compensation: $143,000+
