We’re working with a forward-thinking healthcare organization delivering value-based primary care to senior patients in a supportive, community-centered setting. Clinics are thoughtfully designed to feel welcoming and accessible, with built-in resources that reduce barriers to care and allow clinicians to focus on what they do best: practicing medicine.

Patients receive transportation assistance when needed and access to complimentary wellness programming that supports physical, social, and emotional health–leading to better engagement and more meaningful continuity of care.

What Makes This Role Different

• Average ~12-15 patients per day
• Capped patient panels to protect quality and balance
• Monday-Friday, 8:00 AM-5:00 PM schedule
• No call required (stipend available if interested)
• Dedicated clinical and administrative support team assigned to you

Compensation & Benefits Overview

• Base salary starting around $120,000+
• Up to 10% annual bonus tied to value-based performance
• Total earning potential $150K+

Position Snapshot

• Support: Medical Assistant, scheduling, referrals, and leadership support

The structure is designed to reduce administrative burden and burnout while allowing for deeper patient relationships and better outcomes.

Qualifications

• Nurse Practitioner or Physician Assistant
• At least 2 years of outpatient primary care experience
• Interest in geriatrics preferred (experience welcome, not required)
• Active, unrestricted state license
• Current DEA registration
