Coverage needs:  

• ASAP- Ongoing for at least 12 weeks 

Shifts:  

• Monday – Friday 8a-5p clinical hours

Setting:  

• Outpatient Value Based Care model  
• Average patients per day: 12-18
• Ages 65 plus 
• Experience with Medicare Advantage or value-based care preferred
• Bread and Butter Primary Care 

Support staff:

• Strong care coordination support

EMR:  

• Epic

Travel accommodations

• Covered by facility

Credentialing timeline:

• Immediate

Requirements:

• Active FL license
• Active BC or BE
