Compensation

• Total earning potential: Up to $312,000
• Annual bonus up to 20% of base salary
• Bonus based on:
• Patient satisfaction

Practice Details

• Outpatient Primary Care
• Value-Based Care model
• Patient population: Seniors with 3-5 chronic conditions (heart disease, diabetes, hypertension, etc.)
• 15-18 patients per day
• Maximum panel size of 650 patients
• Shared call schedule based on market provider count (example: 1:32 weeks if 32 physicians)
• Dedicated support team for each physician, including:
• Medical Assistant
• Scheduler
• Referral Specialist
• Leadership support

Requirements

• MD or DO
• Board Certified or Board Eligible in Family Medicine or Internal Medicine
• Geriatrics fellowship preferred, but not required
• Current, unrestricted medical license
• Current DEA registration
