The Role

• Manage a panel of high-risk patients via telehealth
• Conduct assessments, follow-ups, and care planning
• Support transitions of care and reduce readmissions
• Collaborate with nephrology and interdisciplinary teams
• Provide ongoing patient education and longitudinal care

Ideal Candidate

• Active California Nurse Practitioner license and Board Certification
• 2+ years of NP experience (Primary Care, IM, Nephrology, or similar)
• Comfortable managing complex patients independently via telehealth
• Experience with chronic care and value-based models preferred
• Strong communication and clinical decision-making skills
