The Role

• Provide comprehensive primary care to senior patients

• Work within a fully integrated care team

• Utilize data and support tools to improve quality metrics

Ideal Candidate

• Board Certified in Family Medicine or Internal Medicine
• Passion for senior-focused or value-based care
• Strong communicator who thrives in a team setting
• Open to VBC (experience helpful, not required)
• Focused on quality, outcomes, and patient relationships
