We’re partnering with an innovative, mission-driven healthcare organization redefining senior-focused primary care. This value-based model emphasizes continuity, accessibility, and outcomes–delivered in welcoming neighborhood clinics designed to feel more like a community than a traditional medical office.

Patients benefit from transportation assistance (Uber/Lyft for those who don’t drive) and no-cost wellness offerings such as fitness, art, and social activities–removing barriers to care and improving overall health.

WHY THIS ROLE STANDS OUT

* 12-14 patients per day on average

* Capped panels (~600)

* Monday-Friday, 8:00 AM-5:00 PM

* Extremely light, shared call rotation

* Your own dedicated care team (MA, scheduler, referral support)

* Care Model: Value-Based Primary Care

* Patient Population: Seniors with multiple chronic conditions

COMPENSATION & BENEFITS

* Base Salary: $130,000+

* Bonus: Up to 10% of base (value-based incentives)

* Total Potential Compensation: $143,000+

* Medical, dental, and vision coverage (family options available)

* Paid parental leave
