- Coverage needs:
- Start: As soon as credentialed
- End: Ongoing
- Shifts:
- Monday through Thursday
- 8 am to 7 pm (1 hour lunch break)
- Can be flexible with schedule
- Setting:
- Community Health Center
- Ages: 5 and up
- Newer clinic: Low volume
- Support Staff:
- 1 Dental Hygienist
- 2 Dental Assistants
- Procedures:
- Restorative (fillings)
- Extractions, root canals, cleanings
- EMR:
- Nextgen
- Board Certification:
- BC Dentist
- Travel:
- Client will cover mileage and lodging
- Credentialing timeline:
- 30-60 days for full privileges
- Requirements:
- Active AZ license and DEA
